Medical Up-Chits

Before Attending NP4 Water Survival / Physiology training you will need to get a medical up-chit.  This form can be attained in one of two ways:

1. If you have access to a military flight surgeon, schedule an appointment for a flight physical.  After the successful completion of the physical, you will be issued an Up-Chit (form NAVMED 5410/2) that clears you for NP4 training as well as duty involving flying as a project specialist.
    
2. If you do not have access to a military flight surgeon, you must see an FAA approved doctor and have him or her complete form OPNAV 3710/18 .  The first page is a questionnaire that you should fill out prior to your exam. Bring the completed form to your Aviation Training Survival Center on the first day of your Swim/Phys training.

Most importantly--  Keep a copy of the up-chit to take to swim/physiology training and fax a copy to Project Specialist Coordinator to put in your official record.

The following is a list of factors that will be used by the Flight Surgeon to determine eligibility for an up-chit:

In order to fly in USN Aircraft, the OPNAV 3710/18 must be completed, signed by your physician, and countersigned by a flight surgeon before an up chit can be granted.

The front of the form is a medical questionnaire. If block 1-11 is checked yes or block 12 is checked no, then complete documentation by your physician of the evaluation, diagnosis, treatments received, and medications used must be provided to the flight surgeon.

On the back of the form is a physical exam:

• Height must be between 60-78" for men and 58-78" for women.
• Weight must be between 100-235 lbs.
• Pulse must be 45-100 bpm
• Blood Pressure must be less than 140/90
• Corrected visual acuity must be 20/20
• Hematocrit must be between 40-52 for men and 37-47 for women
• Urinalysis must be negative for glucose, ketone, and protein
• EKG must be normal
• All blocks on the exam must be normal

If these standards are not met, complete documentation by your physician of the evaluation, diagnosis, treatments received, and medications used must be provided to the flight surgeon.

Waivers are considered on an individual basis, but require complete documentation.
